Please complete the following steps to register your child(ren) for the upcoming Fall 2013/Spring 2014 soccer season. .
1. Please visit
www.eayso.org and register your child as a player. Print out two of these player forms per child.
If you are having trouble with this step please contact us at aysoregion74@att.net and give us your childs name and birthdate and let us know if they have played in our region before or not.
2. Print and fill out the following attachment. You need
one of these per child playing soccer.Even though you did this once you must do this again.

This attachment also comes with the player form when you complete step 1 so you will not need to print it again in step 2.
4. If your child is new to AYSO Region 74 a copy of the birth certificate must be supplied.
Once you have all the paper work completed you have three options on how to register your child. If you do not include all the paper work your child will not be registered.
Option 1 (make sure you have all the paper work please)
Mail in all paper work to our post office box. Please add extra postage as it will cost more than a normal letter. Post marked by June 1st

Option 3 (please make sure you have all the paper work) By the end of the last game on Saturday, June 1st.
Bring
all your paper work to the last set of games (May 30, May 31, or June 1) You can bring them up to the concession stand and ask for Sherry Evans, Dawn Knedgen, or Matt Galer.

Cost for one player $90
Cost for two players from the same family $170
Cost for three or more players from the same family $240
After June 1st, the price for registration is listed below
Cost for one player $110
Cost for two players from the same family $210
Cost for three or more players from the same family $300
